NOTICE OF NOMINATIONS
This notice is to inform you that per the UDW Constitution, approved by homecare provider delegates elected to the UDW Convention in March 2015, nominations for the following offices are now open.

The chairperson elected by each district shall serve as an UDW Executive Board member by virtue of their election to their district office.
ELIGIBILITY TO RUN FOR PRINCIPAL AND DISTRICT OFFICES:
A. Principal Offices of President, Vice-President & Secretary-Treasurer:
- A member must be in good-standing for two years immediately preceding the election
- (Paid dues monthly from March 2014 to February 2016 ).
- Must have worked at least forty (40) hours per month for the three (3) months preceding the election (December 2015, January 2016, and February 2016).
- Or if on the first six (6) months of an approved leave of absence, provider member must have paid leave of absence (LOA) dues monthly. A member who is on their second approved LOA will no longer be eligible to vote or run for any elected position.
B. For the Board Positions:
- Must be a provider member in good-standing with the district – defined as having paid dues every month for one (1) full year prior to the election (Paid dues monthly from March 2015 to February 2016).
- Or if on the first six (6) months of an approved leave of absence, provider member must have paid leave of absence (LOA) dues monthly. A member who is on their second approved LOA will no longer be eligible to vote or run for any elected position.
- Eligible members are permitted to run for both Principal and District Officer
NOMINATION PROCESS:
- To be eligible to nominate for any of the offices listed above, you must be a union member who signed a membership authorization card prior to November 30, 2015 and paid dues in the month of November 2015.
- All Nominations shall be made in writing using ONLY the form attached to this notice.

Or faxed to 1-916-244-2688No walk-in nominations will be accepted. - Nomination forms must be received by fax, email or mail no later than 9am Friday, January 8th, 2016. All nominees will be notified in writing of their eligibility. All eligible nominees will be posted on the union’s website (www.udw.org) on January 19, 2016.
- The following will be the required information on the nomination form:
- Name of Nominee and Nominator as it appears on their State of California payroll check
- Bargaining Unit of the Nominee and Nominator
- Position for which the member is being nominated
- The name of the Nominee’s Slate (if applicable)
- Provider ID #
- Self-Nomination is permitted.
- Any member may check his/her dues payment record. Dues inquiries must be in writing and mailed to the Elections Committee C/O Clarissa Gonzalez at 4855 Seminole Ave, San Diego, CA 92115, or faxed to 1-916-244-2688.
- Walk-in nominations WILL NOT be accepted.
- Members who have been nominated will be notified in writing of their eligibility to run for office. To the nominees, further instructions on elections will be included in a separate mailer.
- Members nominated for more than one Principal and District Office position must accept one nomination for each office. Acceptance forms will be included with the candidate’s notice of
nomination and must be returned by email, fax or mail to the Elections Committee by 6pm January 18th, 2016. No walk-ins will be accepted. - If a member nominated for multiple offices fails to indicate which office he or she wishes to seek, the Elections Committee will assume the member is running for the highest office for which the member was nominated. If there is only one nominee for an office, that unopposed candidate shall be declared elected.
CHALLENGES TO THE ELIGIBILITY OF NOMINEES
A list of candidates nominated will be posted on the union’s website (www.udw.org) on January 19, 2016. Any member or nominee may challenge the eligibility of a nominee. Any challenges to
the eligibility of a nominee should be submitted in writing to the Elections Committee C/O Clarissa Gonzalez at 4855 Seminole Ave, San Diego, CA 92115, faxed to 1-916-244-2688 or emailed to
ElectionsCommittee2016@udw.org. Eligibility challenges must be received by 4:30 pm on Friday, January 29th, 2016. The decisions of the Elections Committee concerning all eligibility challenges are final.
Nominees will be notified of any challenges as we receive them. Nominees may reply to any protest by Thursday, February 4th, 2016 in writing to: Elections Committee C/O Clarissa Gonzalez at 4855 Seminole Ave, San Diego, CA 92115, faxed to 1-916-244-2688 or emailed to ElectionsCommittee2016@udw.org.
TERM OF OFFICE/INSTALLATION
Candidates elected in the March 2016 officer election will be installed at the first Executive Board meeting held after the tally. The term of office for all positions will end upon the completion of the union’s next regularly scheduled officer election in May 2019.
ELECTION
Ballots will be mailed as early as Monday, February 15th, 2016 but no later than Friday, February 19th, 2016 to all eligible members in good-standing. All members who have signed a membership authorization card prior to January 31, 2016 and paid dues in the month of January 2016 will be eligible to vote in this election. Non-provider staff members can vote for the Principal Officer positions ONLY. Ballots will be picked up from the post office located at 6401 El Cajon Blvd, San Diego, CA 92115 at approximately 11:00 a.m. on Monday, March 21, 2016 and returned to 4855 Seminole Ave, San Diego, CA 92115 for the tally. Each candidate is entitled to name one Official Observer who may be present at both the post office location as well as the tally location. No candidates will be allowed at the count and no candidate may serve as an observer.
Election results will be posted on the union’s website (www.udw.org) after the completion of the tally. If you do not receive your ballot or are in need of a replacement ballot, please contact the Elections Committee at 1-916-554-0931. The last day to receive such a request is at 6pm on Monday, March 14th, 2016.
ELECTION PROTEST
Any member in good-standing may protest the conduct of the election for valid cause. All election protests should be filed with the Elections Committee no later than, 4:30 pm on Friday, April 1st, 2016, in writing and mailed directly to: Elections Committee C/O Clarissa Gonzalez at 4855 Seminole Ave, San Diego, CA 92115, faxed to 1-916-244-2688 or emailed to ElectionsCommittee2016@udw.org. The decisions of the Elections Committee concerning all eligibility challenges are final.
RUN-OFF ELECTION
If no candidate for these positions receives a majority, there will be a run-off election for that office. Ballots for the run-off will be mailed on Monday, April 11th, 2016 and tallied on Monday, May 2nd, 2016.
